Media Release Form  2025/26

 

I grant permission to Lucia Rich/Embodied Arts & Wellness (EAAW) and affiliates or employees of EAAW to use my/my child’s image/photograph for use in media promotional materials including: posters, flyers, email blasts, website, newsletters and social media (IG, FB).

My/my child’s image, photograph will only be used in conjunction with direct advertisement of classes/workshops/events. They will NOT be shared with any third party (with the exception of affiliates of EAAW that are also promoting classes/programs for EAAW). No names of students will ever be included with photos. 

 

I hereby waive any right to inspect or approve of the finished photos or electronic matter that may be used now or in the future, whether that use is known to me or unknown and I waive any right to royalties or any compensation arising from or related to the use of the image.

I acknowledge that I am the parent or legal guardian of the child registered for this class. I have read this release before checking the box on the registration form and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions prior to signing and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.

Note: In the current landscape of online advertisement, is very helpful to use photographs and short videos to promote the continuation of our programing. But we understand that there is sensitivity in this area for parents. If you DO NOT wish to have photographs of your child's face taken OR prefer to have only hands, feet, or back of head used, please check the box saying that you have viewed the waiver and then add a note in the additional information section of the registration form specifying your preferences.
