Kids Release and Waiver of Liability 2024

I, (the parent or legal guardian) am choosing to allow my child/children to participate in Yoga or movement classes/workshops offered through Lucia Rich Khosrowpour/Embodied Arts & Wellness (hereafter referred to as EAAW). 

 

I (the parent or legal guardian) understand that Yoga or movement classes require physical exertion and that my child/children must have adequate physical and mental health to participate. I represent and warrant that they are physically fit enough to participate and they have no medical condition or disability that would prevent or limit their full participation in this class. 

 

I acknowledge that my child/children’s enrollment and subsequent participation in this class is purely voluntary and is in no way mandated.

 

I acknowledge that participation in Yoga or movement classes/workshops exposes my child/children to a possible risk of personal injury. I further acknowledge that these classes/workshops may cause and/or aggravate an already existing physical injury or medical condition. I understand that my child/children may experience muscle, back, neck or other injuries as a result of my participation in classes through Lucia Rich/EAAW.

 

I (the parent or legal guardian) understand my child’s/children’s physical limitations. I am fully aware of the risk, injuries or damages, known or unknown, that might incur as a result of participating in Yoga or movement classes/workshops and hereby release Lucia Rich/EAAW and its affiliates, officers, directors, shareholders, members and employees from any and all liability, negligence or other claims arising from or in any way connected with my child’s/children’s participation in Yoga or Movement classes/workshops.

 

My signature further acknowledges that I (the parent or legal guardian) shall not now or at any time in the future bring any legal action against Lucia Rich/EAAW and its affiliates, officers, directors, members and employees and that this waiver is binding on me, my heirs, my spouse, my child/children, my legal representatives, my successors and my assigns. 

 

My signature (as signified by checking the box on the previous form) is binding to this liability waiver from this day forth.
